Case Management

Private Services. If a client decides to enter into an ongoing relationship with Amore Senior Support Network, we can provide Private Case Management services.
Customized to Your Needs. Private Case Management services are customized to the needs of the client and family. Services can be modified as needed.
Confidentiality. All information is handled with the utmost confidentiality. We are HIPAA compliant.
No Long-Term Contracts. There is an hourly fee charged for services, and a simple pay-as-you-go contract. You are not locked into a long-term contract.

Plan of Care Implementation. Assistance with implementing a plan of care, including coordinating, monitoring, and modifying services being provided by other agencies to our client.
Medical Appointments and Follow-up Care Assistance. Assist with coordination of medical appointments and follow-up care.
Transitional Care Coordination. Assist with a smooth transition back home after a hospital or rehab stay.
Personal Advocate. There are times when we all need someone to come alongside us and help ensure our voice is heard. Examples:
- Do you need someone to help you get return phone calls from service providers?
- Would it be helpful to have someone help you sort out complicated billing issues?
- Sometimes it can be helpful to have an experienced representative supporting you to ensure you can successfully speak openly and honestly with your medical provider, own family or other loved ones. It can be beneficial to have a professional, unbiased third-party to help navigate through difficult conversations.
Benefits Applications Assistance. Assist you with the application process for ALTCS or Veterans Benefits.
Personal Concierge. Since each person has unique needs, there are many more services available. The important thing for us is to give you a customized experience with only the services you need and want.
Contract work. We can be a great addition to your team if you have needs within your organization or community that can use some extra contracted assistance to address.
