Home Visit Assessment

 

One of our staff members will make a home visit and spend as much time as warranted to better assess your situation. Meeting face to face is always wonderful. It is so much easier to get to know someone in person rather than over the phone. Sometimes visiting with you in your home provides us with the awareness of some issues you might be dealing with that are easily forgotten during a phone conversation. We are frequently able to come up with helpful suggestions because we have seen how everything is working for you in your normal surroundings. Our approach to an assessment is to put you at ease while having a pleasant and productive conversation during a relaxed, enjoyable visit. We encourage you to invite a trusted friend or loved one to join us for this assessment. We are your guest, and it is an honor to have been invited into your home to meet you. Since we will be driving to your home and spending as much time as you need, there is a reasonable fee for this service in order to provide you with above and beyond personalized assistance. We will NEVER try to push any services. Our clients' privacy is of the utmost importance, and we are HIPAA compliant.

Plan of Action

 

An in-home assessment will allow us to compile a customized suggested plan of action for care. Relevant information, resources, and referrals to accompany the plan of care will be provided. This, of course, is included in the fee for the home visit assessment.
